All patients scheduled for an appointment will be notified within 24-48 hours of such appointment. we ask that each patient arrive at least 20 minutes prior to the appointment to verify and/or update patient information.
No Show / Cancellation Policy
All appointments require a 24-Hour advance notice for cancellation, we understand emergencies arise, but appreciate your consideration of this cancellation policy. If three such occurrences take place, you may be dismissed from the care of your provider. Failure to present at the time of your visit or give a 24-hour advance notice may result in a “No Show” charge.
Please bring your medications, (both prescribed and over-the-counter) to your appointment visits. It is important to us, and to you, to keep your medications and any allergies updated in your medical records.
Laboratory / Radiology Results
lf you have any laboratory or radiology tests performed, please ask your provider at the time of your visit when you will be notified of the results. lf you have not received either a written or verbal response within 10 days, please call the office.
Patients play a vital role in the success of their healthcare by actively participating with the provider and staff in their treatment. By contributing and adhering to a prescribed treatment plan, the patient increases his or her chances for a successful outcome.

MEDICAL AID IN DYING
In November 2016, Colorado voters passed the Colorado End-of-Life Options Act, which allows qualifying terminally ill adults to obtain a lethal dose of medication to voluntarily end their lives. Under the Act, healthcare providers, including Foresight Family Physicians, are not required to assist qualified patients in ending their lives, and providers are required to establish policies supporting their positions.
Foresight Family Physicians is committed to providing the best possible care for patients approaching the end-of-life, or actively dying, which includes curative and comfort care. We provide coordination for palliative and hospice care, and strive to mitigate suffering and address the fear of pain and loss of control that patients may experience.
We believe we can provide compassionate care and comfort to our patients so they can live with dignity until the time of natural death, and we have therefore opted out of participation.
Foresight Family Physicians’ caregivers will continue to provide coordination of other requested end-of-life and palliative care services to patients and families, and they may discuss the range of available treatment options to ensure patients are making informed decisions with respect to their care.
Any of our patients wishing to request medical aid-in-dying medication will be offered an opportunity to transfer to another facility of the patient’s choice.
