J. Dale Utt, DO has been practicing medicine in Grand Junction since 1979. He graduated from Chicago College of Osteopathic Medicine (now a part of Midwestern University) and is Board Certified by the American Academy of Osteopathic Family Physicians. His interests include all areas of family medicine from children through geriatrics, Osteopathic Manipulation Therapy, Hospitals and sees patients regularly at the local nursing homes.
Dr. Utt has active privileges at Community and St. Mary’s Hospital.
He was appointed to the Colorado Medical Board by Governor Bill Ritter in March 2009, was reappointed in April 2010, and then he completed his Medical Board term in 2014. He participates in the Rocky Mountain Pharmacy Committee and the Continuing Medical Education Committee at Community Hospital is an ongoing interest.
Dr. Gregory C. Reicks joined Foresight Family Physicians in July of 1989. He is board certified by the American Academy of Family Practice. He completed his medical training at the Kirksville College of Osteopathic Medicine in Kirksville Missouri and completed his family practice residency at St. Vincent Hospital in Jacksonville Florida. His special interests include treatment of chronic musculoskeletal conditions and using health information technology to improve the quality and efficiency of health care delivery.
Dr. Reicks is on active staff at the both Community Hospital and St. Mary’s Hospital. In addition, he serves as the president of the Mesa County Physicians IPA as well as president of the Quality Health Network, our regional health information exchange.
Heather Chynoweth, PA-C was born and raised in the Grand Valley and had the privilege of growing up in a healthcare related family. Seeing her grandfather’s ENT practice, her grandmother working as a Psychiatric Nurse, mother as a Physical Therapist and father as an epidemiologist. This background has helped to form her belief in the benefits of a Medical Home, which is the incorporation of multiple medical providers to treat the patient as a whole. Heather completed her undergraduate degree in Biology at Mesa State College in 2000 and went on to Midwestern University in Glendale AZ, where she received a Master’s Degree in Medical Science and Physician Assistant studies. She returned to the Grand Valley and started practicing at Foresight Family Physicians in the spring of 2004 and at Mesa Otolaryngology in 2008. Heather enjoys working with all ages and feels the patientÂ provider relationship is the key to maximizing preventative healthcare. She has many interests in medicine and enjoys the Family Practice specialty. Other interests include volleyball, biking, hiking, camping, gardening, cooking, traveling, and spending time with family.
Glenda Cain joined Foresight Family Physicians in January of 2015. Glenda received her Bachelor of Science in Nursing degree from Mesa State College in 1994, then her Masters and FNP degree in 2003. For the last 10 years Glenda has been doing Locums (traveling) and has worked in a variety of settings with experience in several areas of specialty practice around the country. She is interested in community projects such as health fairs, domestic violence, sexual assault programs, as well as local church projects. She was named Volunteer of the Year and was the top fundraiser for a diabetes walk in 2003. Glenda has 5 children and has been a foster parent to several youngsters. She knows exactly what it’s like to be a parent.
Kathleen joined Foresight Family Physicians in Jan 2017; she has a Doctorate of Nurse Practioner, from the University of Utah, Cum Laude, and Primary Adult/Gerontology. She has been a Neurological Trauma Staff RN at St Mary’s, Program Director at CMU, Procedure RN at Community Hospital, and an RN coordinator at Marillac Clinic. Kathleen‘s focus will be our adult patients at Foresight Family Physicians. She has 2 adult children and enjoys the many outdoor opportunities that the Grand Valley offers with her family and friends.
Hailey attended Colorado State University in Fort Collins, Colorado, where she graduated with a degree in Sociology and minor in Criminal Justice. She completed three years of Health and Exercise Science education and for the past three years has studied Addiction Counseling and Health Coaching. Hailey has worked closely with various population including Substance Use, Severe and Persistent Mentally Ill, Chronically Homeless and General Mental Health.
Hailey Joined Foresight Family Physicians in May 2017, as our Behavioral Health and Wellness Counselor. Her interest include, going on adventures with her son, camping, hiking, enjoying the outdoors, and spending time with family and friends.
Ginny is our Integrated Health professional. She is part of the health care team meeting with patients before or after their appointment with their provider. She is also available for longer appointments as needed. She is both a Registered Dietitian and a Licensed Professional Counselor. Ginny obtained her undergraduate degree in nutrition and dietetics from Western Illinois University. While living in Colorado she obtained her Master’s in Public Health from the University of Northern Colorado. After a move to Illinois she earned her Master’s in Clinical Psychology from Benedictine University. She has worked in hospitals, residential facilities, group homes and community mental health centers. In her off time she enjoys horseback riding, camping, water color and quilting.
Dr. Shannahan (Pamela) is our Integrated Health Professional. She is part of the health care team meeting with patients before or after their appointment with their primary care provider. She is also available for longer appointments as needed. Dr. Shannahan holds a BS degree in Human Service Management from University of Phoenix; MS degree in Marriage, Family, and Child Therapy from University of Phoenix; and a Doctoral in Behavioral Health from Arizona State University. She specializes in Early Childhood Education/Development; Infant and Early Childhood Mental/Behavioral Health; Marriage, Family, Child Therapy; Integrated Behavioral Health with focus from birth to death. She is a Licensed Professional Counselor with the State of Colorado. Dr. Shannahan has worked in educational settings, community mental health settings, primary medical care offices and law-enforcement victim services. In her off time she enjoys travel, family, and photography.
Kathy Olsen, RN, MGH, MHA returned for the second time to Grand Junction 22 years ago. She obtained a bachelor of science in Nursing from Biola University in 1981, and has since worked in a variety of different nursing venues including hospital, public health, hospice, insurance and care management. Blessed to be in leadership most of her career, she pursued master degrees in Health Administration and Geriatric Health from A.T. Stills University in 2009. She came to work at Foresight Family Physicians in 2016. Passionate about helping people, she finds joy in assisting our patients with understanding their disease processes and treatments, as well as connecting them with appropriate community resources.
In Kathy’s free time, she loves to spend time with her family and friends. She adores her grandkids and makes every effort to spend time with them! She loves to tinker in her tiny garden, bead, hike, learn to crochet, and read. She finds joy and hope in her faith, and gratitude for the work that she is privileged to do as a care manager.
Jennipher started at Foresight Family Physicians in 1994 as a part time registered medical assistant for Dr Reicks. She left briefly to focus on her family, then rejoined the Foresight team in 2009 and was asked to join the Quality Improvement team. As an active Quality Improvement team member, she is involved in several initiatives that are working hard to improve our patient’s health and to lower healthcare costs, and facilitates the Patient and Family Advisory committee. This is where her love for care management started. Jennipher’s goal as Dr Utt’s Care Manager is to be a catalyst, educator, and problem solver to his patients. She enjoys the challenge of encouraging patients to be more involved in their own healthcare and educating them about what our community has to offer. While she is out of the office, Jennipher loves to spend time with her family and friends, paddle boarding in the summer and snowboarding in the winter.
